Inserting questionaire.

Hi. I am looking to create a questionaire maybe using 'coffee cup' or html software and insert into a Media Studio file which i can email out to people who can then complete it and return it to me with their answers marked. i would like to give them questions which they can rate with a number scale next to each question, maybe using 'radial' buttons which they can click on.

does anyone know if this is possible? and maybe give me some advice on how to go about it?

i was thinking of letting the user give grades and/or answers to your questions. then save thoses answers to a ini file and let your app mail it back to you
