Form Image Software

I've got a customer (a Canadian government agency, eh?) that wants to use my exam software, but with a twist. The test item is to be an EXACT image of a form. They are going to present a scenario and show the form on the screen. The examinee is supposed to check entries on the form (with a little check box ON the form) that will then send stuff back to my program for analysis.

Does anyone know of a nice chunk of form creation software that I could shell out to for retrieving information from a form? I'd like some sort of gizmo that would be generalized enough so that as they come up with lots of forms it's not too terribly painful to stick a new form on the screen.

I was going to recommend Acrobat, which can handle the form creation side, but not the data interchange thing. So.... As Stewie might say, "Blast!"

I was going to say the same thing. Worm posted a document for Acrobat 7 that exposed the COM interface. You could use LuaCOM to create the form but I would think that you could return information from the form as well.

What about a web page with a form and when you submit, you return the URL to AMS for analysis?

Looks like there are some Adobe / VB data-extract gizmos out there I can get. I've got a couple of leads, but before I get too excited I sent a note to the customer saying "It's Going To Cost...". I'll see if they are still interested.

Now... I, for my own nefarious reasons, am STILL interested, so if you think of anything, I'd still like to hear!

Well, I may be misunderstanding, but, here is a thought:

Just have a screen shot of the form(s) then put them into an Image object. Overlay some Hotspot's or Input object's to capture the checks inputed by the end-user.
