How are old files removed?

I have two versions with lots of outdated files in the old release. The new release does not contain these files any more.

Why are those files not beeing removed when applying a patch?

Am I just missing to enable the option or is Visual Patch not able to delete those files?

Please do not reply to use file commands, I do not like to run file commands here as this is very complicated in my situation!
