How to Create Popup Alert Dialog Box

Dear all
I need to create a pop-up dialog box that is displayed randomly every 2-5 minutes. The dialog box needs to contain text, buttons and the PC system time. I have the Popup Notify plugin but do not know how to create the above functionality.
Any examples or help is appreciated.
