Username and Password login screen problem

Hello Im making an application for our network staff and I will like to have a main login screen like the one at the attachment. After the username and password is confirmed I will like to redirect to the second screen. Is there anyone here that have made a project with a similar login screen?

Sure, I have. I guess others have too. But you dont tell us how you are checking the password or what the problem is you are experiencing.
