Dr. watson

I have made a application, and I have a problem when run in a windows NT OS. The Dr. Watson appears with this following message.
An application error has occurred and an application log is being generated.
safra.exe ( Auto Play exe)

Exception: in page io error ( 0xc0000006), address 0x00453f34

Please, help-me to resolve these problem.
